SCS Purchase Order Templates
SCS PO Templates:
This will seem time consuming, but the benefit of being able to use the templates when adding POs to CMS will far outweigh the work to create the templates. The template feature is only available on our Premium CMS.
Early Supports – create a template for every level of provider that you have in your team 1-5 and label accordingly:
DNAs and closure notice will be for 12 months when you are creating the actual POs for your clients. Please ensure you enter the start and end date when completing these service codes.
If you don’t, that line will use the default date range.
TASK: EARLY SUPPORTS PLAN
Cover and Wellbeing Plan - create a template for every level of provider that you have in your team 1-4 and label accordingly:
NB: If this is the first PO for this client for the SCS contract, also add 6 x DNAs and 1 x Closure Notice – for 12 months
TASK – COVER & WELLBEING PLAN
Short Term Support to Wellbeing - create a template for every level of provider that you have in your team 1-4 and label accordingly:
Task – Completion Report
Specialist Cover Assessment - create a template for every level of provider that you have in your team 2-5 and label accordingly:
When creating the PO, you can then add the lead provider lines if applicable. This example PO is intended for a standalone assessment (hours are for a client who is not having therapy with your provider) You can create the template to suit your own practice.
TASK - Specialist Cover Assessment Report
Wellbeing Plan Post Cover - create a template for every level of provider that you have in your team 1-5 and label accordingly:
I have added DNAs x 10 and a Closure notice x 1 – this will be for 2 years.
NB. As we move further into the coming months, you may wish to remove the DNAs and closure being on the template as a matter of course – new clients moving through from Early Cover and not needing an assessment will probably still have valid DNAs – you would then add the new allocation to the Pathway A or B POs.
TASK – WELLBEING PLAN
Package A Tailored Support - create a template for every level of provider that you have in your team 1-4 and label accordingly:
NB. There are extra steps for this type of PO – only for A and B package/pathway POs
When you create the PO – add the type – Pathway A or B – this will assist with reporting
I suggest that once you start adding secondary providers to this PO that you assign each individual line to the correct provider – this will prevent incorrect billing.
As you add service codes for secondary providers, please ensure you deduct those hours from the lead provider allocation – using MAX ALLOCATION will prevent hours going over what has been approved and can be used for the DNAs as well if the lead provider is sharing those too.
Please bear in mind that I have shown the max hours ever approved for Package A – you will need to replace with what has been approved. This PO is for 2 years (unless the transition only allowed for 1 year) Click into max allocations – enter the total amount of hours for SCSWA to match what has been approved and do not add a suffix.
Tasks for Package A & B as below:
The instructions for Package B are the same – just replacing the code and type, with the addition of Level 5 for Package B.
Maintaining Wellbeing - create a template for every level of provider that you have in your team 1-6 and label accordingly:
This PO is for 3 years. No task
